Dictate the note. Get it back finished.

The documentation does not get shorter and the day does not get longer, so it moves to the evening. Dictate at the end of the visit, by phone or recorder, and the structured note comes back in your template ready to review — rather than waiting for you at nine at night.

Where the clinic day goes

Time in the EHR per weekday, per clinical full-time equivalent.

Total EHR time5.9 hrs
During clinic hours4.5 hrs
After clinic hours1.4 hrs

Source: Arndt et al., Annals of Family Medicine, 2017 — out of an 11.4-hour workday. Documentation alone was 84 minutes of it.

How it runs

Two passes before it reaches you

The second pair of eyes is the whole product. Speech recognition alone will confidently write the wrong dose.

From dictation to signed note

Per note
Step 1You dictatePhone, app or handheld recorder — whichever fits how you already work.
Step 2Secure uploadAudio moves over an encrypted channel. Nothing sits on a device.
Step 3TranscribedBy someone who knows the vocabulary of your specialty.
Step 4Clinical proof-readA second pass for terminology, drug names and dosages.
Step 5FormattedInto your template — SOAP, consult, operative, discharge.
DoneInto your EMRDelivered by EMR integration or secure transfer, ready for your signature.
Anything the transcriptionist could not hear cleanly comes back flagged rather than guessed. A blank marked for your attention is safe; an invented drug dose is not.
The evidence

The burden is measured, not anecdotal

Peer-reviewed research, not OnPoint RCM client results.

The split between patients and paperwork

27 percent of physician time is direct clinical face time and 49 percent is EHR and desk work27%patient face time

Roughly two hours of EHR and desk work for every hour of direct patient care. Documentation is the largest single piece of it.

Source: Sinsky et al., Annals of Internal Medicine, 2016 — time-and-motion study across four specialties. EHR and desk work: 49.2%.

What documentation support changes

Charts closed in 48 hrs, physician-drafted28.5%
Charts closed in 48 hrs, scribe-drafted32.6%

From a randomized trial of scribes. Physician satisfaction with time spent charting improved dramatically; patient satisfaction was unchanged.

Source: Gidwani et al., Annals of Family Medicine, 2017 — randomized controlled trial, unadjusted figures. Transcription is not identical to scribing, but the documentation burden it addresses is the same.

Questions we get

Before you ask

Is this speech recognition or a person?

A person transcribes and a second person proof-reads. Recognition software is fast and it is also confidently wrong about drug names, dosages and anything said quickly — which is exactly where a documentation error hurts.

How does the note get into our EMR?

By direct integration where your EMR supports it, or by secure transfer into the folder your staff already work from. Either way it arrives formatted, not as a wall of text to reshape.

Who has access to the audio?

Only the assigned transcriptionist and the proof-reader, under HIPAA-compliant workflows and a signed business associate agreement. Audio is not stored on personal devices and is retained only as long as your policy allows.

What if the dictation is unclear?

It comes back flagged for your review rather than filled in with a best guess. We would rather hand you a blank to correct than a plausible invention to miss.

See what your practice is leaving on the table

A 30-minute call. Tell us roughly how many Medicare patients you see, which EMR you use and what you already run in-house. We come back with which programs your patients qualify for, what each one would take to run, and what it is likely to be worth.
