Practice service · included with billing

The monthly file you can act on.

Most billing reports say what happened. The useful ones say why, and what changed since last month. Every denial we work is grouped by root cause and counted, so the pattern shows up as a number you can do something about rather than a stack of individual appeals.

The measurement loop

What each month’s report is for.

Claims measured against payer editsFirst pass
Denials grouped by why, not by payerRoot cause
Fix applied upstream, then re-measuredNext month

Metric definitions follow HFMA MAP Keys, the standard the industry uses, so your numbers mean the same thing as everyone else’s.

How it runs

A report that changes something

If a number never causes anyone to do anything differently, it does not belong in the file.

Measure, group, fix, measure again

Monthly
Step 1Data pulledClaims, remittances and denials for the month, straight from your system.
Step 2Metrics computedFirst-pass acceptance, days in A/R, ageing buckets, collection rate.
Step 3Denials groupedBy reason code and root cause — eligibility, registration, coding, authorization.
Step 4Cause identifiedWhich step in the cycle produced the problem, and how often.
Step 5Fix applied upstreamChanged at registration, coding or authorization — not re-fought per claim.
DoneRe-measuredNext month shows whether the fix worked. If it did not, we say so.
A denial category that does not fall after a fix is reported as a fix that failed. The point of measuring the same things every month is that it becomes obvious when something is not working.
What is in it

Three questions, answered the same way every month

Consistency is the feature. Comparable months are what make a trend visible.

Revenue cycle healthFirst-pass acceptance, days in A/R and ageing buckets, net collection rate, and what moved since last month.
Denials, by whyGrouped by root cause rather than payer, with the count, the dollars behind it, and the recovery rate on appeal.

What we do, and what stays with you

OnPoint RCM does

  • Pulls the data and computes the metrics
  • Groups every denial by root cause
  • Attributes each cause to a step in the cycle
  • Recommends the upstream fix, in plain language
  • Re-measures next month and reports whether it worked
  • Flags anything moving the wrong way before you feel it

Your practice keeps

  • Every underlying record — it is your data
  • The decision on which fixes to adopt
  • Direct access to the work queues behind the numbers
  • The right to have us show our working on any figure
Questions we get

Before you ask

Is this an extra cost?

No. It comes with billing. A billing service that cannot tell you why claims are failing is not really doing the job.

Can we get the raw data?

Yes, and you should ask for it. It is your data and it lives in your system. Any figure in the report can be traced back to the claims behind it.

How is this different from the reports our software already produces?

Your system reports what happened. It rarely groups denials by why, and it never tells you which change to make upstream. That interpretation is the work — and next month says whether we got it right.

See what your practice is leaving on the table

A 30-minute call. Tell us roughly how many Medicare patients you see, which EMR you use and what you already run in-house. We come back with which programs your patients qualify for, what each one would take to run, and what it is likely to be worth.
